Clinical chart

Patients who need a copy of their medical records, of reports from outpatient clinic visits or emergency ward visits, of x-rays (x-rays, CT, MRI) must contact the Front-office for copies of clinical records (Sportello unico copia documentazione clinica).

The Office is located on the ground floor of the Rizzoli Hospital (via G.C. Pupilli 1, 40136 Bologna). It is open to public Monday to Friday from 9.00 a.m. to 12 a.m.

The office gives general information and information regarding when the documents are ready by phone (ph. +39-051-6366343) Mondays to Thursdays from 8.30 a.m. to 10.30 p.m.

Procedure for copies

Request for a copy of Medical Records must be requested:

  • at the Front-Office;
  • by fax at the number +39-051-4689918
  • in writng addressed to:

When the Front-Office is closed, a form, available at the Office, must be filled in and placed in the Office mailbox. 

The request must clearly and comprehensibly included: 

  • Type of copy: report of visit, clinical record, x-ray (specifying which type x-ray, CT, MRI); 
  • X-rays in vision for inspection; 
  • Personal Data (surname and name, place and date of birth); 
  • Telephone number and address where the copy must be sent; 
  • The request must be signed and a copy of a valid document included; 
  • For a minor the parent or guardian must declare custody;
  • For a  deceased patient, the person filing the request must declare that he is a relative or heir.

Procedure for collection

Copies of clinical records (clinical records, reports from outpatient clinic visits or emergency ward visits, x-rays: x-rays, CT and MRI) must be collected directly at the Front-Office (after confirmation by phone to be sure they are available):

  • by the patient (after having shown a valid document);
  • by a person delegated in writing (after having shown their document and that of the patient also in copy); 
  • WARNING! For a minor the parent or guardian must declare that he has custody.


For x-rays performed before 31/01/2007: Copy or vision of the x-rays taken at our hospital both for visits (outpatient clinic and emergency ward) and/or during hospitalization may be requested.
For x-rays performed after 01/02/2007: X-rays taken during hospitalization may be requested in copy (on x-ray or CD) or in vision.  X-rays taken during a visit (outpatient clinic or Emergency ward) may be requested only in copy of x-ray or CD.

  • X-rays in vision are collected directly at the Front-Office (they are not sent by mail).
  • All postage and payment of delivery costs of copies of clinical records must be paid by the recipient.

How to pay

The user receives a payment bill with a barcode, with which he/she must go to the automatic cashier available at the Institute, in order to pay for the requested documentation. The payment can also be made online, by following the instructions available on the bill itself.
